PACE Lafayette Program of All-Inclusive Care for the Elderly is a nonprofit health and supportive services program deed to assist seniors who want an alternative to long-term care placement. With PACE Lafayette's assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance; physical relief and emotional support. PACE Lafayette takes a team approach to care which revolves around its diverse, multi-disciplinary team.

Eligibility Requirements PACE participants must: Be 55 years of age or older Meet the criteria for admission to a nursing facility Be able to live in the community with PACE Lafayette support without jeopardizing participant's health or safety Live in Lafayette, Carencro, Breaux Bridge, Broussard, Youngsville, Scott, Duson, Rayne and Sunset with Zip Codes of,,,and Receive all needed healthcare, including primary care and specialist physician services other than emergency servicesfrom PACE Lafayette or from an entity Adult service Lafayette Louisiana by PACE Lafayette.
If at any time you are permanently placed in a nursing home, you may be responsible for a portion of the cost of your care.
